To Make An Appointment

For existing patients

If you are an existing patient of Dr. Browne, simply ring the office on 087 1530121.

New Referrals

If you feel you need referral to the rheumatology clinic, please approach your GP who will be able to refer you if they feel it is appropriate. Referrals from other healthcare professionals are also accepted.

Why do I need a letter of referral from my GP?

It is a recommendation of the Medical Council of Ireland that people wishing to attend a consultant medical specialist should be referred by their GP. We regret that we are unable to see people without referral. The referral letter is also an important source of information about your background medical history, investigations relevant to your problem and helps us greatly in prioritising the urgency of your medical problem. How long does it take to be seen?

Appointments are offered quickly on receipt of a referral letter. In the event of an emergency, people can be seen very quickly.


Presenting For Your Appointment

Each week we run a number of different clinics. Please select the appointment you are attending for and see the information relating to that appointment.

New Patient

1. Once your appointment has been made you will receive a text message with the appointment details. Twenty-four hours prior to your appointment you will receive a reminder text. If you want to cancel or reschedule please contact the office at 087 1530121.

2. If for any reason you cannot attend please notify the office so that we have time to offer the appointment to another patient. We will happily reschedule you.

3. These appointments are carried out in Dr Browne’s private rooms in the Kerry Clinic, located in the grounds of the Bon Secours Hospital Tralee.

4. Dr Browne’s room is located upstairs in Suite 15.

5. Please arrive to your appointment at least 5 minutes ahead of schedule. Please be mindful that if you are late you will not be seen. Punctuality is very important to ensure the clinic runs on time.

6. When you arrive you do not need to report to the secretary.

7. Take a seat in the blue chairs on the landing and Dr Browne will call you when he is ready.

8. It is perfectly acceptable to bring a friend or loved one with you to an appointment, many people do find this quite beneficial. Obviously, any persons present during a consultation will be party to personal details in relation to your healthcare.

9. Wifi is available in our waiting room.

10. Appointments for new patients are estimated to last 30 minutes.

11. New appointments cost €200. This is payable on the day by cash, card or cheque.

Review

10. Biologic appointments are estimated to last 45 minutes. During this time you will first meet with the Exercise Referral Specialist where you will receive an exercise advice and educational programme. Some exercises will be practiced during this time and you should dress appropriately in comfortable footwear and loose clothing. This will take 20 minutes. You will then meet with Trish, the rheumatology nurse who will carry out a number of checks and discuss any concerns with you. Dr Browne will also be in attendance. This will take 25 minutes.

11. Prior to receiving a biologic treatment it is important that your bloods are checked. The required blood tests can be carried out on the day in the Bon Secours Hospital at a cost payable by you. Trish will then contact you if there are any issues. Alternatively, you can have your bloods done with your GP 2 weeks prior to your appointment and you can bring these results with you on the day. The required bloods are: FBC, U+E, LFT, CRP and ESR.

Important Biologic Clinic Information

Biologic medicines (or Bio’s for short) are used in the treatment of various arthritic conditions. They are called biologic medicines because of the way they are made and developed.

Prior to commencing treatment with a biologic you will require the following blood tests:
FBC, U&E, LFT. (€34)
Hepatitis B & Hepatitis C (€52)
QuantiFERON (€ 49)

Your GP can carry out the blood tests in BLUE however the blood test in ORANGE must be carried out in a hospital. If you decide to get the blood tests done in the Bon Secours Tralee the associated cost is indicated next to the test. There is a phlebotomy fee of €9 euro also each time you use the Bon Secours Lab. If you would like to have these blood tests carried out in the Bon Secours Tralee please let us know and we will put the request through for you.

While you are receiving this treatment you will need to be reviewed in clinic at regular intervals to assess how your treatment is working for you. This review period will be agreed at the end of every appointment but cannot be any less often than every 6 months.

Prior to your review appointment you will need to have some bloods repeated: FBC, U&E, LFT, CRP and ESR (€52 +€9 phlebotomy fee). These need to be completed within 3 months of your appointment. You should have these blood results with you on the day and/or ask your GP to fax the results to Dr Browne’s Office (066 7167182) and they will be added to your file.

Your appointment on the day will last approximately 1 hour. During this time you will first meet with the Exercise Referral Specialist where you will be assessed and receive exercise advice. Some exercises will be practiced during this time and you should dress appropriately in comfortable footwear and loose clothing. You will then meet with Trish, the rheumatology nurse who will carry out a number of checks and discuss any concerns with you. Dr Browne will also be in attendance.

Your prescription will be given to you at the end of your appointment, please give this to your pharmacist.

Appointment cost is €80, which is payable on the day by cash, card or cheque.

Bone Health Follow Up

Important Bone Health Clinic Information

The “Bone Health” clinic is where patients who are on treatment with teriparatide (Forsteo or Movimya) are assessed and managed.

The treatment duration with Teriparatide is 2 years. During this time you will have 4 appointments.

Appointment 1: Your first appointment will last approximately 1 hour. During this time you will first meet with the Exercise Referral Specialist where you will be assessed and receive exercise advice. Some exercises will be practiced during this time and you should dress appropriately in comfortable footwear and loose clothing. You will then meet with Trish, the rheumatology nurse, who will discuss the medication with you in detail, answer any queries you may have and demonstrate administration of the therapy. Dr Browne will also be in attendance. During this appointment you will receive your first 6 month prescription.

Appointment 2 & 3: Six months later your next appointment will be held over the phone with Trish. Your progress will be discussed and any concerns reviewed. After each call a 6 month prescription will be issued. Two weeks prior to this appointment you should have the following bloods repeated and ask your GP to fax the results to Dr Browne’s Office (number on top of page): U&E, Calcium.

Appointment 4: Your last appointment will be approximately 1 hour. During this time you will again meet with the Exercise Referral Specialist followed by Trish and Dr Browne. Your last 6 month prescription will be issued. A plan for further treatment after discontinuation of teriparatide will be put in place.

Appointment cost is €200 euros (this is a one off payment at the beginning of treatment and covers all of the above)

Infusion Clinic

First Infusion

1. To be commenced on an infusion you will have met with Dr Browne and discussed this in detail.

2. You will be contacted by the Infusion Clinic on the phone and/or in writing and you will be given a date and time to attend for your infusion.

3. Your infusion will be taking place in the Bon Secours Hospital Tralee.

4. When you arrive to the hospital make your way to the Outpatients Department in the main hospital.

5. Some paper work will be completed with you and you will then be directed to the Infusion Clinic.

6. In the infusion clinic you will meet the nurse and your infusion will be administered. How long the infusion takes is dependent upon what treatment is being administered. The nurses in the Infusion Clinic will be able to tell you how long the infusion will take.

7. Dr Browne and Trish are available if there are unforeseen issues during the infusion.

Subsequent Infusions

1. When you arrive to the hospital make your way to the Outpatients Department in the main hospital. Again some paper work will be completed with you and you will then be directed to the Infusion Clinic.

2. In the infusion clinic you will meet the nurse and your infusion will be administered.

3. Dr Browne and Trish are available if there are unforeseen issues during the infusion.

Injection clinic

1. To be eligible for an injection clinic appointment you must first have been reviewed by Dr Browne or have been referred by another clinician specifically for an injection.

2. Type of injections performed: All joint areas except spinal joints. Carpal tunnel injections and Sacroiliac Joints (SI Joints) are injected. Epidural injections, nerve blocks and spinal facets cannot be injected in this clinic. We can refer you to a service where these can be performed upon request.

3. Once your appointment has been made you will receive a text message with the appointment details. Twenty-four hours prior to your appointment you will receive a reminder text. If you want to cancel or reschedule please contact the office at 087 1530121.

4. The procedure is carried out in the Main Hospital. You will be given an appointment time and you should report to the Admissions Department at that time and no earlier. Your name will be called, you will complete some paper work, once completed you will wait to be called for your injection.

5. The procedure itself is quick and minimally invasive. The nurse will prepare the area that is being injected. Dr Browne may use Ultrasound to guide the injection if necessary. If fluid is present, it may be drained and if necessary, can be sent away for analysis. The injection will be performed. You can expect the procedure to last approximately 10 minutes. Once complete you can leave the hospital.
Please note that there will be no consultation during this appointment. If you want to discuss anything in detail with Dr Browne you will need to arrange a clinic appointment.

6. Please allow 2 hours for the entire process. We will endeavour to have you out as quickly as possible but it is a very busy clinic and delays do happen.

7. We recommend that you do not drive after the injection. It is advisable for you to have other arrangements made.

8. Patients with health insurance may have to pay an excess on the day up to a maximum of €150. This is dependent on the specifics of your policy. You can enquire with your provider whether you are covered for procedure code 4334 and if an excess is payable. The code for carpal tunnel injections is 3184.

9. Patients without health insurance can also have an injection at a cost of €150 per injection.

 


Prescreening Bloods

Prescreening bloods are very helpful and can greatly assist in the establishment of a diagnosis at the time of your appointment. If you bring bloods along with you, a more comprehensive assessment can be made.

Typical pre-screening bloods are outlined below. If you write these down to bring to your GP they will understand what they mean.

1. Rheumatoid Arthritis, Psoriatic Arthritis, Seronegative Arthritis, Inflammatory Arthritis: FBC, U&E, LFT, ESR, CRP, CK, Ca, anti-CCP antibody, RF.

2. Fibromyalgia, Chronic Pain: FBC, U&E, LFT, ESR, CRP, CK, TFTs, B12, Folate, Iron, Ferritin, Ca.

3. Gout: FBC, U&E, LFT, ESR, CRP, Urate.

4. Low Back Pain (for longer than 3 months): FBC, U&E, LFT, ESR, CRP, Ca, Ig&SPEP.

5. SLE (lupus), Sjogrens Syndrome, Scleroderma, Connective Tissues Diseases: FBC, U&E, LFT, ANA, ENA, C3,C4, dsDNA, ESR, CRP
